Comprehensive Initial Evaluation
$495 · 90 minutes
Your first clinical appointment, scheduled once your intake and laboratory results are in.
We arrange the time with you directly, so the appointment is spent on findings rather than gathering information.
Follow-Up Visit
30 minutes · established patients
For patients already enrolled in Ongoing Clinical Management.

Calls are answered when we are available. If we are with a patient, leave a message and we will return your call within two hours during office hours. Messages left after hours or at weekends are returned the next business day.
Text messages are welcome for scheduling and general questions. Please do not send medical information by text.

Where care is available
Raven is a telehealth practice. Appointments are held by secure video visit for patients physically located in New York State at the time of care. Raven does not maintain a patient-facing office.
We serve eligible adults throughout New York State, from Buffalo and Rochester to the Hudson Valley, Long Island, and the North Country.
